DLA Publications Information Request Form

To order DLA Conversion Schedules and DLA Customer Assistance Handbooks for use by DLA customers, please complete the following information below:

Information on the Handbook is available to DLA customers on the publications page. 

Please complete ALL required fields in order for the form to be processed successfully which includes entering "0" (ZERO) in the products you do not want.

Postal regulations require full address information for delivery. To ensure proper delivery, please provide the full agency name, street address, and mail stop, building number, or office symbol (if applicable).
