Description: This form serves as the agreement between employees and their supervisors once participation in the telework program has been approved. DLA has gone electronic through EAGLE Telework Management and is available only to DLA civilians at this time. However, paper version of this form must remain active for Military and NAF personnel who want to apply to participate in the program.

Records Retention: 5300.19 Telework Program. Destroy 1 year after end of employee’s participation in the program, or after superseded form becomes obsolete.
