Description: This form updates and redesignates DSCR Form 2995 to a DLA-level form as a result of the 2019 forms survey on MSC-level forms. The DLA Aviation Safety Office uses this form for the purchase of safety shoes for DLA Richmond employees and DLA Aviation Forward site employees until December 2021. Once their contract ends, organizations will start purchasing safety shoes for their employees, ending the need and use of this form.
