Description:	The Procurement Technical Assistance Program (PTAP) is a cooperative agreement program executed by the Defense Logistics Agency (DLA), Office of Small Business Programs (DLA-DB). Responsibilities for monitoring and reporting performance under Federal financial assistance awards (e.g., cooperative agreements) are stipulated in 2 CFR §200.328. This form will be used to capture performance reports from cooperative agreement recipients under the PTAP.
